Have you lost or misplaced your SWATA CEU certificate?

If you have misplaced your SWATA CEU certificate, you can request a replacement copy from the SWATA District Secretary, Lorna Strong. Please give the following information in your email:

1. Full name
2. NATA member number
3. Address
4. Year of lost certificate

The District Secretary will verify your attendance at the meeting and send you a replacement certificate. Certificates will be emailed to you as an Adobe pdf file and you can print it for renewal purposes. Please allow 2 weeks for processing of replacement certificates.
